This master thesis deals with psychiatric care and social welfare of mentally disordered, and accordingly, psychiatric ill adolescent persons. This care is characterised by interfaces between different institutions. Especially in adolescent phase of life mental disorders, respectively, psychiatric diseases might become chronicle, which additionally might increase due to an inappropriate care. Adolescents concerned have often already been in long-term care, accompanied by numerous interruptions of relationships, and therefore require a goal-oriented, continuous care. A new approach of attendance known as ?Case Management? is presented in this master thesis. The status quo of problems and chances of interdisciplinary crisis intervention are shown by means of research of the theoretical background of case management and a survey of empirical sections of the qualitative approach based on expert interviews.
